Founded in 1964, UMOM is Arizona's larger provider of services for families experiencing homelessness. Our mission is to prevent and end homelessness using innovative strategies and housing solutions that meet the unique needs of each family and individual. With an annual operating budget of 22 million dollars, UMOM provides shelter, housing, and supportive services to youth, families, and individuals experiencing homelessness in Maricopa County. UMOM serves just over 12,000 unique individuals across all our services annually, including those families receiving support at the Family Housing Hub.

Position Description: The Specialist, SSVF Housing Stability will provide support and resources to veterans and their households in order to obtain and/or maintain permanent housing in accordance with the Support Services for Veteran Families grant as administered by the VA.


Essential Duties and Responsibilities:

1      Provide screening, intake, and assessment for all incoming SSVF families.

2      Work with community coordinated entry to receive referrals for SSVF program

3      Utilize SPDAT assessment to assess functional and social need of clients in order to develop, implement and monitor service plans leading to an increased level of self-sufficiency.

4      Determine and access appropriate community, state, federal health and social resources, upgrading and enhancing a community resource database.

5      Provide Case Management in-office and in-home visits on an ongoing basis and administer temporary financial assistance as needed by clients.

6      Respond to client needs in a timely manner.

7      Monitor client progress on an ongoing basis.

8      Maintain client confidentiality.

9      Provide outreach to community regarding resources available through the SSVF program.

10   Assess housing needs of clients and develop a housing case plan.

11   Develop and maintain a list of housing providers and relationships with housing providers in Maricopa County.

12   Guide clients through the process of maintaining or obtaining permanent housing.

13   Provide education to clients in areas of tenant rights and responsibilities.

14   Document and maintain accurate information in client files on a timely basis.

15   Comply with established policy and procedure.

 Schedule: Must be available to work Mon-Fri with two late shifts or Tues-Sat with one week day late shift.


  • Bachelor's degree in Social work or related field preferred. High School diploma required
  • Minimum of one year direct case management experience
  • Desired Knowledge of community resources and understanding of community health and social service system
  • Current AZ Driver's license with no more than 2 moving violations in a one year period
  • Requires 50/100 level of auto insurance (mileage reimbursement available)
  • Level One Fingerprint Clearance Card or the ability to obtain one
  • Knowledge of current Windows OS, MS Word, Excel



Experience working with veterans, knowledge of Rapid Rehousing, Trauma Informed Care and Motivational Interviewing.

Work Environment:


While performing the duties of this position, the employee travels by automobile and is exposed to changing weather conditions. The employee works in varied residential environments based on assigned caseload.
