UMOM New Day Centers of Phoenix, AZ is actively seeking a full-time Housing Case Manager to assist clients in accessing and maintaining permanent housing through UMOM's DES funded Rapid Rehousing program. Founded in 1964, UMOM is Arizona's larger provider of services for families experiencing homelessness. Our mission is to prevent and end homelessness using innovative strategies and housing solutions that meet the unique needs of each family and individual. With an annual operating budget of 22 million dollars, UMOM provides shelter, housing, and supportive services to youth, families, and individuals experiencing homelessness in Maricopa County. UMOM serves just over 12,000 unique individuals across all our services annually, including those families receiving support at the Family Housing Hub.

Essential Duties and Responsibilities:

Direct Client Care

  • Confirm eligibility, complete intake and assess barriers to housing stability and determine housing needs collaboratively with household
  • Create Housing Stabilization/ Case Plan individualized to each household and based on assessment of client needs and update at each client meeting based on completion of goals and ongoing needs.
  • Address barriers to employment for all households, create employment goals and assist households in obtaining and maintaining employment.
  • Assess household rental subsidy on a monthly basis.
  • Provide monthly updates to supervisors regarding stability of all households.
  • Provide a minimum of 4-6 hours of direct client care per month including in-person home visits.
  • Support household in identifying and accessing benefits and resources to maintain housing stability including coordinating care on behalf of the client.
  • Serve as liaison between landlord and household and assist with any tenancy issues that may arise.

Housing Search and placement

  • Schedule and accompany households to housing briefing appointment.
  • Assist households in identifying appropriate housing options including provision of transportation when necessary to meet the unique needs of the household.
  • Accompany households to meet with landlords and/ or contact landlords on behalf of households to explain Rapid Rehousing Program and advocate on their behalf.
  • Assist households in asset mapping of resources in vicinity of new permanent housing.
  • Work in collaboration with RRH household, shelter case manager and/ or navigator to ensure continuity of care and rapid exit from shelter/ homelessness to permanent housing


Documentation, Data Quality and Outcomes

  • Assure that data is complete, accurate, and on-time in accordance with UMOM's data quality standards.
  • Focus on achieving agency outcomes of exits to permanent housing, increased income, data quality and program efficiency.



  • Experience and education:

    • Bachelor's degree in Social work or related field or equivalent experience preferred.
    • Desired Knowledge of community resources and understanding of community health and social service system.
    • Demonstrated experience working with people experiencing homelessness or other vulnerable populations required. May include work experience, volunteerism or internship.
    • Case Management experience strongly preferred.


    Computer skills:

    • Proficiency with applications including Microsoft Word, Excel, Power Point, and Outlook



    • Valid Level One Fingerprint Clearance Card or the ability to obtain one
    • Valid AZ driver's license and a driving record that falls within UMOM's policy
    • Vehicle insurance in accordance with UMOM driving policy.

Knowledge of community resources and an understanding of community health and social service system is a plus!


Must be available to work two late shifts during the weekday on a Mon-Fri Schedule, or one late shift and a day shift on a Saturday on a Tuesday-Saturday Schedule.
