Founded in 1964, UMOM is committed to our mission to prevent and end homelessness using innovative strategies and housing solutions that meet the unique needs of each family and individual. At UMOM, we move quickly to place families or individuals into short-term, subsidized housing, and then deploy a range of supportive services to overcome barriers to permanent housing. We provide safe shelter and supportive services for over 170 homeless families. We also offer nearly 300 units of affordable housing across the valley, each with special program centers for residents.

Position Description:  The Specialist, Resident Support works with families in the permanent supportive housing programs to identify barriers to maintaining permanent housing, help families establish goals to address these barriers, and support the family in achieving these goals in order to maintain safe permanent housing.


Essential Duties and Responsibilities:

1      Provide intake for all new permanent supportive housing residents and accompany to briefings when appropriate.

2      Assess functional and social need of clients in order to develop, implement and monitor service plans leading to an increased level of independent living.

3      Determine and access appropriate community, state, federal health and social resources, with focus on maintaining stable housing

4      Work diligently with property management, HOM.Inc, and other UMOM staff to assure residents maintain housing.

5      Provide additional services such as transportation, or accompany client to access community resources as necessary to assure stable housing

6      Provide outreach to community resources and maintain and enhance resource and referral database.

7      Engage clients in services on ongoing basis minimum of weekly but preferable 3-5 contacts per week

8      Serve on call for resident emergencies

9      Provide resources and referrals for residents with goal of maintaining housing

10   Monitor client progress on an ongoing basis.

11   Assist in developing program policy and procedure

12   Participate in ongoing client staffing

13   Maintain client confidentiality.

14   Document and maintain accurate information in HMIS and client files on a timely basis.

15   Comply with established agency policy and procedure



  • High School Diploma required. Bachelor's Degree in Social Work or related field preferred.
  • 1 year of direct case management experience required.
  • Valid AZ Driver's license with no more than 2 moving violations in a one-year period required.
  • Level One Fingerprint Clearance Card or the ability to obtain one required.
  • 50/100 level auto coverage insurance required.
  • Knowledge of current Windows operating system, MS Word and Excel
