UMOM New Day Centers is actively seeking a full-time Family Shelter-Case Manager!

As a member of a Family Support team, the Family Shelter Case Manager works with families in the Emergency Shelter programs to identify barriers to permanent housing, establish goals to address these barriers and monitor and support progress toward these goals until permanent housing is obtained.

Founded in 1964, UMOM is committed to our mission to prevent and end homelessness using innovative strategies and housing solutions that meet the unique needs of each family and individual. One of these innovative strategies is our Rapid Rehousing program which is an effective approach that is in use nationwide. We started our program over ten years ago and it is still experiencing growth. At UMOM, we move quickly to place families or individuals into short-term, subsidized housing, and then deploy a range of supportive services to overcome barriers to permanent housing. We provide safe shelter and supportive services for over 170 homeless families. We also offer nearly 300 units of affordable housing across the valley, each with special program centers for residents.

Essential Duties:

  • Provide intake and assessment of all clients of the UMOM New Day Programs, including an explanation of program rules and policies.
  • Assess the functional and social needs of clients to help guide the case planning process with the family, which will lead to the best housing option.
  • Monitor the case plan with each family, making changes as needed to respond to client needs and client exit planning.
  • Actively engage all families on caseload, with face to face meetings at least once per week and as often as needed to achieve income and permanent housing goals.
  • Maintain up to date client files, with timely and accurate progress notes for each family receiving services. Maintain client confidentiality and follow program guidelines for client charting.
  • Maintain timely and accurate ETO and HMIS data entry according to program policy and procedure.
  • Respond in a timely manner to newly assigned clients and current client needs.
  • Collaborate with UMOM Workforce and Housing teams on client goals for successful exit.
  • Work closely with other members of the family support team to provide quality services to all families.
  • Participate in weekly team meetings for purposes of supervision, training, and staffing
  • Utilize evidence-based practices in service provision, including Motivational Interviewing.
  • Maintain current information relative to city, county, state and federal health and social service resources.
  • Complete all required reports according to stated deadlines.
  • Perform community outreach to acquire referrals from outside agencies.
  • Coordinate with outside service providers to assist clients with meeting case plan objectives.
  • Provide community-based services as needed to families per Critical Time Intervention Model, if appropriate.
  • Comply with established agency policies and procedures

 

QUALIFICATIONS TO BE A FAMILY SHELTER CASE MANAGER

  • High School Diploma/GED required. Bachelor's Degree in Social Work or related field preferred.
  • 1 year of direct case management experience required.
  • Experience with victim services or domestic violence survivors preferred.
  • Valid AZ Driver's license with no more than 2 moving violations in a one-year period required.
  • Level One Fingerprint Clearance Card or the ability to obtain one required.
  • Ability to work flexible hours, including two late nights or weekend days per week.
  • Knowledge of current Windows operating system, MS Word and Excel.
