Founded in 1964, UMOM is committed to our mission to prevent and end homelessness using innovative strategies and housing solutions that meet the unique needs of each family and individual. At UMOM, we move quickly to place families or individuals into short-term, subsidized housing, and then deploy a range of supportive services to overcome barriers to permanent housing. We provide safe shelter and supportive services for over 170 homeless families. We also offer nearly 300 units of affordable housing across the valley, each with special program centers for residents.

Position Description:

The Outreach and Engagement Specialist (OES) will follow a progressive engagement strategy to empower youth experiencing homelessness to strengthen and expand opportunities available to them, including improving their housing situations. This position is designated half time to the Phoenix Youth Resource Center and half time to Street Outreach.

 

Essential Duties and Responsibilities:

 

Phoenix Youth Resource Center (YRC)

  1. Provide efficient, friendly customer service to clients, volunteers, guests, and staff members visiting the center. Ensure all visitors record their visit using the Visitor Log.
  2. Greet visitors to the center in a welcoming and trauma informed way, communicating clearly that intention of services and staff are to help resolve homelessness.
  3. Assist youth access basic needs services such as showers, ready-to-eat food, hygiene products, and clothes closet and laundry. Set clear intentions with youth that these services are part of a strategy to resolve their housing crisis.
  4. Engage with youth, encouraging involvement in center programming, groups, and services.
  5. Provide support to the development team in the event of donations to assist in equitable distribution of resources to clients.
  6. Maintain a safe space for participants to communicate openly and without judgement, while still following the state laws of mandated reporting.
  7. Support and aid volunteers and interns assigned to the Phoenix Youth Resource Center.

 

Street Outreach and Mobile Resource Center

  1. Practice best practices for street based outreach, outreaching in teams and ensuring teammate is accounted for and safe. Maintain awareness of surroundings.
  2. Maintain and develop positive working relationships to maximize services to street based youth, helping them to access resources in their community of origin if possible.
  3. Load van for Street Outreach, ensuring availability of basic needs.
  4. Participate in strategy sessions to identify and target areas where street based youth congregate. Outreach should be targeted, intentional, and planned.
  5. Provide information regarding the YRC and encourage participation to resolve homelessness
  6. Coordinate with other regional outreach teams to ensure regional coverage to street base youth.
  7. Practice a Harm Reduction and Trauma Informed approach to youth encountered on Street Outreach, being explicit with intent to resolve housing crises

 

Best Practices

  1. In collaboration with the Youth Engagement Coordinator, help plan for continuous improvement of client centered, strengths-based engagement for youth experiencing homelessness, visiting the Phoenix Youth Resource Center, and/or accessing youth engagement services.
  2. Provide immediate problem solving and safety planning, including access to housing and shelter, to youth and their companion pets. 2 Outreach and Engagement Specialist 07/18/17
  3. Conduct screening interviews, triage basic needs, and complete intake documentation.
  4. Survey youth and young adults for housing services, using the common tool prescribed by the community for coordinated entry.
  5. Perform assessment for potential victims of crime, labor and/or human trafficking, and connect youth to services and safety planning as well as connection to legal services, advocacy services or law enforcement when necessary.
  6. Assist clients in stabilization and self-sufficiency, assisting to identifying what supports are needed and identified by client in the housing preparation process.
  7. Assist youth gather documentation essential for housing.
  8. Provide assistance and referrals to education, employment, and housing resources.
  9. Provide support and advocacy to youth when needed.
  10. Coordinate with OES team, Coordinator, and Director as needed to assist in service connection and identify any unmet needs. Research and share awareness of the communities expanding resources.
  11. Participate in multidisciplinary case staffing sessions to ensure a unified approach to service delivery.
  12. Connect youth under 18 years of age to Safe Place services.
  13. Use de-escalation techniques as taught by Crisis Prevention and Intervention training.
  14. Maintain confidentiality and appropriate personal and professional boundaries.
  15. Maintain electronic and case records in a prompt and proper manner, with daily documentation that is objective and concise.
  16. Participate in a data driven and outcomes based culture. Programs should consistently demonstrate effectiveness and efficiency. Ensure timely entry of all statistical information and required data.
  17. Provide support, as directed, to the Evening Reporting Center program.
  18. In collaboration with the Coordinator and Director, seek opportunities for youth feedback and evaluation of the program on a quarterly basis.
  19. Demonstrate and model Trauma Informed Care, Harm Reduction and cultural competency. Actively participate in in-service trainings (including Motivational Interviewing, Diversion, SPDAT, CPR, and CPI), regular staff meetings and available community trainings.
  20. Understand and comply with established agency and program policies and procedures.
  21. Actively pursue continuous professional growth to promote excellence and innovation within UMOM. Stay current in relevant content areas, further develop leadership expertise, network and collaborate, and refine management skills. Set annual goals for self-care, professional development, and emotional intelligence.
  22. Work collaboratively with Youth Engagement Coordinator to create and implement a professional development plan and meaningful documentation of performance. Meet with supervisor weekly, more if needed, for professional development, training needs, program development, and ensuring services delivered are trauma informed and youth and families are engaged. Work with supervisor to gain support in establishing rapport and a professional relationship with youth, including establishing and maintaining appropriate boundaries.
  23. Actively work towards "One UMOM" integration in which programs are seamless for participants, staff work across program and department lines, and communication is open, transparent, and inclusive whenever possible. Collaborate with partners throughout the agency to increase efficiency, effectiveness, and consistency of UMOM programs. Maintain and develop positive working relationships to maximize services to street based youth, helping them to access resources in their community of origin if possible.
  24. Implement service delivery that aligns with national best practices and UMOM values. Insure that YES daily interaction with youth is infused with basic life and coping skills.
  25. Strategize with Engagement team and Director to overcome special challenges unique to this service population. Research and share awareness of the community's expanding resources.
  26. Actively participate in in-service trainings, regular staff meetings and available community trainings.
  27. Create and foster a healthy, collaborative, and safe work culture.
  28. Coordinate with Coordinator and Director for any literature for distribution that represents UMOM.
  29. Elevate any issues of liability, health and safety or legal risk to Manager. Actively report any inappropriateness witnessed to the proper authorities.
  30. Comply with agency, local, state, and federal policies and procedures.

 

Qualifications:

  • Bachelor's Degree in Social Work or related field preferred.
  • 1 year of direct case management experience required.
  • Extensive knowledge of community resources.
  • Experience working with youth and young adults.
  • Valid AZ Driver's license with no more than 2 moving violations in a one-year period required.
  • Level One Fingerprint Clearance Card or the ability to obtain one required.
  • 100/300 level auto coverage insurance required.
  • Proficient knowledge of current Windows operating system, MS Word and Excel.
