Founded in 1964, UMOM is committed to our mission to prevent and end homelessness using innovative strategies and housing solutions that meet the unique needs of each family and individual. At UMOM, we move quickly to place families or individuals into short-term, subsidized housing, and then deploy a range of supportive services to overcome barriers to permanent housing. We provide safe shelter and supportive services for over 170 homeless families. We also offer nearly 300 units of affordable housing across the valley, each with special program centers for residents.

Position Description:

The Family Support Coordinator reports to the Manager, Family Shelter and is responsible for supervising the case management team made up of Family Support Specialists and overseeing the delivery of the services provided to all families assigned to the team.

Essential Duties and Responsibilities:

  1. Liaison with service providers to provide continuity of care for families through appropriate community referrals
  2. Conduct regular team meetings, to assign clients to staffing.
  3. Engage in the daily supervision of Family Support specialists, including direction of work, appraisal of productivity and efficiency, delivery of feedback and coaching, and disciplinary action, if required.
  4. Engage in training of staff on UMOM policies and procedures
  5. Conduct quarterly all-family team meetings and alternate Campus Council Facilitation with Family Support Coordinators
  6. Conduct and oversee assessments and intervention with agency clients as needed.
  7. Monitor team caseloads and assign specialists to all new family intakes.
  8. Monitor program services, data quality, and performance outcomes, ensuring required documentation and data entry is complete and accurate, using file reviews, HMIS, and other required databases.
  9. Respond to incident reports, complaints and grievances from clients and staff.
  10. Provide supervision to all direct reports, meeting on a weekly basis to help develop strengths, talents, and skills. Document supervision meetings for the purpose of recommending promotions or other changes in status, and retain all documentation in employee files.
  11. Provide limited case management case load coverage, as needed
  12. Review client files on a monthly/quarterly basis and provide guidance and direction to the Family Support Specialist team  
  13. Coordinate with the Family Housing HUB and other shelter and housing providers to determine eligibility for shelter and Permanent Housing options.
  14. Co-facilitate Clinical staffing
  15. Staff client cases with Director of Client-Centered Services as needed.
  16. Comply with established UMOM policies and procedures.

 Qualifications:

  • High School Diploma/GED required. Bachelor's Degree in related field preferred
  • Minimum of 2 years behavioral health/human services experience required
  • Supervisory experience preferred
  • Valid AZ Driver's license with no more than 2 moving violations in a one year period
  • 100/300 level car insurance required (mileage reimbursement available)
  • Level One Fingerprint Clearance Card or the ability to obtain one required
  • Knowledge of current Windows operating system, MS Word and Excel.
