We provide safe shelter and supportive services for over 170 homeless families. We also offer nearly 300 units of affordable housing across the valley, each with special program centers for residents.

Position Description: 

The Case Manager, Women's Center provides support, advocacy, safety and resources to unaccompanied women and human trafficking survivors who are temporarily sheltered with the goal of assisting clients in identifying, obtaining and maintaining permanent housing.

Essential Duties and Responsibilities:

Engagement:

  • Provide diversion services and complete intakes for women entering shelter within 24 hours of shelter entry.
  • Lead intentional housing related conversations with women in shelter daily.
  • Support women in ensuring that their basic needs are met while in shelter, connecting them with community resources based on their unique circumstances to ensure that their basic needs will continue to be met upon exiting shelter.
  • Support women in shelter less than 30 days and those of lower acuity that have stayed longer than 30 days to self-navigate their way out of homelessness.
  • Provide information to the Woman Support Advocates team about: Engaging women in housing focused dialogue.  Tips for finding and maintaining housing.  New affordable housing in the community.  Key information relevant to housing progress for individual women on the Housing Case Managers' case load.
  • Work with the Woman Support Advocates to resolve safety issues and conflict between women.
  • Assist women in planning for their safety based on their unique circumstances (in their new housing, while in shelter, in the community, related to substance abuse, mental health, domestic violence/sexual assault and human trafficking if applicable, etc.) using a harm reduction approach.
  • Collaborate with the Woman Support Advocates to support and encourage each woman to take action around her housing plan.
  • Utilize evidence-based practices in service provision, including Motivational Interviewing and Trauma Informed Care.
  • Assist participants with referrals for behavioral health and substance use services, mainstream benefits, health services, identification and documentation services, domestic violence/sexual assault and human trafficking support groups and services.
  • Assist with identifying and screening human trafficking survivors.
  • Attend community meetings, workshops, conferences and training events related to human trafficking.
  • Assist in educating and training the other staff on human trafficking related issues.

Assessment and Data

  • Ensure that VI-SPDAT for all women entering shelter are completed within 24 hours and entered into HMIS as required.
  • Enter data for new shelter entries into ETO/HMIS within 24 hours of entering the program.
  • Enter case notes into ETO /HMIS within 24 hours of service being provided.
  • Update income and disability (chronic health) in ETO / HMIS as changes occur.
  • Enter initial full FSPDAT and update in accordance with agency procedures.
  • Enter exit data into HMIS and ETO within 1 business day of shelter exit.
  • Close files for women who have exited shelter within 3 business days of shelter exit.
  • Responsible for meeting established performance outcomes; including the percentage. of women on caseload exiting to permanent housing.

Housing-based case management

  • Be accessible, approachable and responsive to the needs of a case load of approximately 25 women
  • Complete full SPDAT assessments within 2 weeks of shelter entry.
  • Assist participant with developing a plan for permanent housing based on the unique needs and circumstances of each woman within 2 weeks of shelter entry. Update as needed.
  • Secure identification and documentation needed to access housing; Responsible for ensuring the all women in shelter longer than 30 days are document ready for housing
  • Write timely and accurate case notes for each woman receiving services. Maintain participant confidentiality and follow program guidelines for charting.
  • Transport and accompany women in shelter to search for housing, meet with property managers, complete housing applications, research and access necessary resources in the community and become familiar with local resources in the community in which they will be housed.
  • Provide education to women about tenant rights and responsibilities.
  • When exiting women to permanent housing, provide orientation to the neighborhood and ensure connections with contacts and resources, inclusive of but not limited to local viable transportation options
  • Assist women exiting shelter to permanent housing with developing a plan to sustain their housing so that at least 80% do not return to homeless services system within 2 years of their exit date.
  • Provide community based services as needed to women per Critical Time Intervention Model.
  • Staff each participant with the team for progress and support.

Other

  • Research local housing providers and affordable housing on an ongoing basis and maintain an updated list of local housing providers and affordable housing options
  • Attend staffing, team meetings and ongoing trainings as scheduled.

Qualifications:

  • Bachelor's degree in Social work or related field preferred.
  • 1-year experience working with human trafficking survivors, domestic violence survivors, at risk youth 18 -24 and /or case management required.
  • Ability to work flexible hours, including one evening per week and one weekend per month
  • Desired Knowledge of community resources and understanding of community health and social service system.
  • Knowledge of current Windows operating system, MS Word and Excel
  • Valid AZ Driver's license with no more than 2 moving violations in a one-year period required.
  • 100/300 level auto coverage insurance required.
  • Level One Fingerprint Clearance Card or the ability to obtain one
  • Successfully pass maximum risk and background screening as per City of Phoenix requirements this shall include felony and misdemeanor records from any county in the United States, the state of Arizona, plus any other jurisdiction where the Contract Worker has lived at any time in the preceding seven (7) years from the Contract Worker's proposed date of hire. Plus a sexual offender search for the preceding seven (7) years and driving record search for the preceding seven (3) years from the Contract Worker's proposed date of hire.
