Founded in 1964, UMOM is committed to our mission to prevent and end homelessness using innovative strategies and housing solutions that meet the unique needs of each family and individual. One of these innovative strategies is our Rapid Rehousing program which is an effective approach that is in use nationwide. We started our program over ten years ago and it is still experiencing growth. At UMOM, we move quickly to place families or individuals into short-term, subsidized housing, and then deploy a range of supportive services to overcome barriers to permanent housing. We provide safe shelter and supportive services for over 170 homeless families. We also offer nearly 300 units of affordable housing across the valley, each with special program centers for residents.

A DAY IN THE LIFE OF A HOUSING CASE MANAGER


As a Housing Case Manager, you are able to manage your own caseload and most of your time is spent interacting with clients and responding to their needs in a timely manner. You work with Family Housing Hub to receive referrals for Rapid Rehousing families and assess and document their eligibility. You assess the ongoing housing needs of UMOM residents seeking Rapid Rehousing and develop a housing case plan when necessary. Once residents are a part of the Rapid Rehousing program, you assure their ongoing eligibility for the program and that all program fees or rent subsidies are paid on time.


You build relationships with housing providers in Maricopa County and develop and maintain a list of those housing providers. This allows you to assist families in identifying housing and complete all necessary compliance procedures while educating them in areas of tenant rights and responsibilities. You guide clients through the process of maintaining or obtaining permanent housing.


In order to update and enhance our community resource database, you determine and access appropriate community, state, federal, health, and social resources. You assess the functional and social needs of clients in order to develop, implement, and monitor service plans leading to an increased level of self-sufficiency for clients in the Rapid Rehousing program. You provide case management home visits to all Rapid Rehousing clients and monitor their progress on an ongoing basis. You document and maintain accurate information in client files and maintain client confidentiality. As part of the future of homeless services, you love that you make a difference in the lives of homeless youth, individuals, and families.


QUALIFICATIONS TO BE A HOUSING CASE MANAGER

  • At least a year of direct case management experience
  • Current Arizona driver's license with no more than 2 moving violations in a one year period
  • 100/300 level auto coverage insurance
  • Working knowledge of Microsoft Office
  • Level one fingerprint clearance card or the ability to obtain one


A bachelor's degree in social work or a related field is preferred, but not required. Knowledge of community resources and an understanding of community health and social service system is a plus!


WORK SCHEDULE


The typical schedule for this position is Monday through Friday from 9am to 5pm, but it is flexible. The schedule you will work depends on what best meets your own schedule and the needs of your clients.
